Where can I find information about the selection criteria used by US grad schools during admissions?

I would like to know the GRE/TOEFL and GPA cutoff scores that are used by various grad schools.Also do top grad schools insist on projects or paper publications?What are the chances of getting selected in a top school with 1500+ gre score but with no projects?
